PAVO is seeking to recruit to the following post:-
Community Connector - Base will be in Hay-on-Wye or Talgarth working 28 hours per week (Flexible working considered)
The post holder will support adults, as part of integrated community based health and well-being support services, to access local community services and activities and services provided by the public, private and third sectors, by "brokering" their access to these services and working with the individual to find bespoke solutions to meet their personal needs. They will support people to access the right support, first time, in the right way and seek to empower them to build their own networks of support within their community.
Salary:- £19,971.20 per annum (£24,964 pro rata) funded until 31st March 2018
